37579635 2002-10-15 CASE 02-048 The Uniontown office received a complaint on 10/11 from Dennis Evans with the Barberton Fire Department concerning a gas leak at the wellhead. Inspector Worstall check the site on 10/11 with Mr. Evans and found that Schreiner had repaired the leak. The site was revisited on 10/15 to confirm the repair and inspect the site. No violation at this time.
489415308 2014-12-11 I was on location to conduct a status check of this well. Review of records in RBDMS indicates the last reported commercial production was in 2006 for this well. The well is of plunger lift construction. All valves at the wellhead are closed. The well is fenced in but the man door was not locked. The wellhead is very overgrown with small trees and shrubs. The tank battery has a 210 BBL production tank, a 1500 gallon poly brine tank and a mechanical separator. The 210 BBL production tank has holes rusted through it about half way down from the top of the tank. The production tank valve has a bull plug. The 1500 gallon poly brine tank was approximately half full of fluid and was missing the top hatch. The chart in the meter box shows no production and the chart was illegible. The tank battery is fenced but the man door was not locked, and the tank battery is overgrown with small trees and shrubs. There is no signage at the well. The access to this well is stable and drivable. The following needs completed by:

1) Locks installed on the man door to the tank battery and on the production tank valve (1-23-15) 2) Install a new production tank and remove the old existing one (2-8-15) 3) Have vegetation removed from around the wellhead and from inside the tank battery (2-8-15) 4) Identification sign posted (1-23-15) 5) Well shall be plugged (7-8-15)
