1414528502 2014-10-28 On site with Inspector Bob Roush and staff from the federal EPA, Air Monitoring Division. Lease road gate and entrance gate were locked. Lease road is stable and in good repair. Pad is stable and in good repair. Wells on pad are in production. Tank battery consists of six 400 BBL production tanks that are connected to a flare. The site has three horizontal GPU units, one vertical GPU unit, a compressor and one oil stabilization unit. Cement barriers are placed in front of the oil and gas wells and the production equipment. No violations noted at the time of this inspection.
-1750266290 2013-02-26 I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole has drilled a 24 hole to a total depth of 115' and ran 20 casing to a depth of 115'. O-TEX cemented the casing with 175 sacks of Class A 2% CaCl; .5 pound per sack cello-flake; .4% C41P. The cement had a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. O-TEX pumped 36.78 barrels of cement slurry and approximately 3 barrels of cement was circulated to surface. NOTE: O-TEX had trouble breaking circulation before they started mixing the cement.
