1313155781 2011-08-12 Witnessed surface casing set and cemented in. 9 5/8 conductor set in 12 1/4 borehole. 454 feet of 7 inch 17 lb. surface casing set in 469 feet of 8 3/4 borehole. 85 sacks Class A, with 50 lb LCM, 200 Gel, 250 Lb accelerator, and 50% excess. Circulation established with good cement returns, cement level stayed at surface.

1321554346 2011-11-17 Preliminary restoration check. Pits filled and drilling equipment removed. Base of pump jack had sunk 6-8 inches in soft soil and entire system was out of alignment. Bridle, and stuffing box were loose, causing a small leak from tubing. Pump jack itself had nearly slid off base. Contacted Bob Adams of American Natural Gas and notified him.
