1362416554 2013-03-04 Status check inspection on the current drilling progress found the current well bore section to be complete with a total depth reached of 8366' using a 12.250 bit drilled on air. The company is preparing to load the hole with fluid and trip out of the hole with the drill string to prepare to run the 9.625 casing string. Inspector will note that all casing permit conditions are being followed.

1373500691 2013-07-10 On site inspection and status check found a service rig on the hole running in with tubing preparing to drill out the frack plugs. I reviewed the flow back procedures and methods of storing well bore produced fluid in the temporally set 500 barrel tanks on site, along with the use of non-explosion proof pumps in the transfer of produced product. The company has broken no Ohio laws we are merely reviewing all procedures in the completion process. Ron Trivisanno and Blake Arthur from the Divisions Engineering section was with me on site.

I was on site and did receive company records from the Conductor and Surface jobs indicating size and length of pipe as well as the dates the casing was set, no records were found recorded in the database, company man indicated that the jobs were witnessed by a inspector.

2092540780 2013-07-29 The company is currently in the process of hooking up the production flow lines on the said well along with the remaining wells on the drill pad. I discussed with the company representatives on site the need to clean up the contaminated material from around the well heads.
2144437239 2013-10-07 Preliminary Restoration found the wells on site all have been drilled and stimulated and the production equipment in place awaiting the gas processing transporting facility to begin purchasing the product. 6- 210 barrel storage tanks are on site in 2 tank individual containment areas, Proper well head identification is in place along with identification at the well site entry point. The site meets ORC 1509.072 regarding restoration.
-309523253 2013-05-13 Status check inspection of the stimulation process found Producers Services on location stimulating the well, A completion plug was set @ 14,714 with the well perferated between 14,655' and 14,414' with 60 perforations. Multiple wells are being completed simultaneously on this pad.
