1108399963 2012-11-02 The unit is comprised of a 2-tank battery (2 210-barrel production tanks). There are four in-ground concrete, brine tanks located adjacent to the production tanks. The wellhead includes a mechanical plunger lift located in a 6-foot chain-link enclosure. The tank battery is shared with the Lassnick #1 Unit. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
-1304428236 2014-10-29 I inspected this location has part of routine inspection activity. The well is producing and production records are current. The fittings and connections at the wellhead are free from signs of leakage or other problems. Fittings at the tank battery are free from signs of leakage or problems. The load-outs are bull-lugged. Secondary containment is intact. ID Signage is present.
1352150713 2012-11-01 The wellhead is fitted with a mechanical plunger lift and is located in the back portion of a private residence. The well is located remote from the tank battery, which is shared with the Lassnick # 1 unit. The well is good condition and the well is not showing any pressure on the 8.625-inch casing. I did not observe any signs of fluid or gas leaks. The lease road is in good condition. There is no ID at the wellhead. I will call Tim Reilly at Great Plains and ask him to put an ID placard at the wellhead.
