-1316081763 2009-04-28 I performed a preliminary restoration status check. I GPS'd both the wellhead and the tank battery for the first time. The drilling rig and debris are gone. The pits are closed. Good vegetation is growing. This well PASSES PRELIMINARY RESTORATION.
-1659722977 2011-09-21 Received call from Don Avi (330-717-6675) on 9/21/2011 in regards to sheen/film on pond. When collecting water sample, the sheen did not break up and left a yellowish-brown residue on collection cup. The residue did not have an oil/gas odor and it dried out quickly when rubbed between fingers. Chloride test results = 87 ppm (mg/L), Ph test results = 6.5. The sheen/film in water sample precipitated to bottom of collection bottle after time. I inspected the well behind Mr. Avi's property. There were no leaks or spills around well head or tank battery and the well was properly identified. I did not observe any spots of dead vegetation or grass between well and pond, and no dead grass around pond. Did not detect an oil/gas odor at any time throughout the investigation. No violations at this time.
-1674224424 2006-12-18 Yost Drilling has finished and is moving off site. All equipment should be removed by 12/19. Wade Pol Contracting will start removing pit water today.
-1850559320 2012-03-13 On 3/13/2012, DNR Inspector Richard Skidmore completed a production well inspection status check for DNR Inspector Chris Grimm. The well head was a pumping unit. The tank battery and well head are located behind the barn. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
-1963126181 2007-01-24 The drilling pits have been solidified and closed, the site has been rough graded, the tank battery has been set and the well is ready to turn in line. Well ID still needs to be posted on the tank battery.
-2088791627 2010-09-20 This status check was requested by Jay Cheslock. I performed a field inspection and walked around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
-555230619 2011-09-22 Returned to further investigate cause of sheen/film on pond surface. Joined by inspector Tom Hill for second opinion. Sheen/film produced yellowish-brown bubbles when water surface agitated. Took samples of water for laboratory analysis, took digital photographs of pond surface and water samples. Strained water through filter to isolate particulates for further examination.
