Production equipment includes two 100bbl tanks within an earthen dike and a horizontal separator. The well is equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with the valves closed on the tubing and one of two valves open on the casing providing domestic gas. There has been no change at the well or tank battery since the last inspection. Photographs were collected. Compliance Notice

1318961438 2011-10-11 Arrived on location and met with landowner ( Earl Scott ) he advised well has been idle for 10 years and would like to have well plugged. Checked production reports and found no reports of production since 1996, within 60 days apply for a plug permit. Temporary inactive status will not be permitted do to length of time well has been nonproductive.
1330440084 2012-02-28 Arrived on location and found well has not been put into production or sold. An extension was given on 11/1/11/ until 01/01/2012 to find a buyer for this well. At this time the Everett Lynn E #1 needs to have a plug permit issued and a plan to plug within 10 days. No extensions will be given and no temporary inactive status will be issued. Well needs to be plugged.
1364996690 2011-11-01 Received call from Roger Proper jr requesting an extension until 01/01/2012 .Blaze oil & gas may have a buyer for this well so it can be placed into production. I gave company an extenison until 01/01/2012.
1364998056 2013-03-26 Arrived on location and found well is still idle, checked file and no plug permit has been applied for as of this date. Landowner wants this well plugged and out of his field since it hasn't produced since 1996. At this time I'm requesting a chiefs order to get this well plugged.

1383570605 2013-11-04 Called Roger Proper jr attorney for Blaze oil he advised he would talk with company and up-date me. I told Mr. Proper if something is not done soon I would apply for a chief's order to plug well.
1401463892 2014-05-30 On the day of my inspection I found the well to be idle. The valves were in the off position. The last year of production was 1995. According to the previous inspections, numerous extensions have been granted to Blaze Oil & Gas to transfer or plug this well. The Last contact with Blaze Oil & Gas was 11-4-2013. As of this date Blaze Oil & Gas still owns this well, and a plugging permit has not yet been applied for. An NOV was issued By Inspector Berry Grimm on 2-28-2013.
1871450416 2014-05-01 On the day of my inspection following up on NOV-1330440084, NOV-1318961438, complaint id #3592 and complaint id #3390, I witnessed that the production equipment included 2-100bbl tanks within an earthen dike and a horizontal separator. The well was equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with the valves closed on the tubing and 1 of 2 valves open on the casing servicing domestic gas. I made call to Blaze Oil & Gas at 330-345-6700 and spoke with Dave Sohl and asked him what the plans for the well was he stated that if he had to plug the well he would probably be filing for bankruptcy due to not having the money to pay for plugging.
732433002 2015-02-09 On the day of my inspection I am following up on NOV-1330440084 and NOV-1318961438. I found that the production equipment includes two 100bbl tanks within an earthen dike and a horizontal separator. The well is equipped with a gas plunger lift system, 1.5 inch tubing, 4.5 inch casing, and 8.625 inch casing. At the time of my inspection the tubing valve and main production valves are closed. The second casing valve is open and is run to a 1 inch orange line which appears to be for domestic use. According to RBDMS, production has not been reported since 1996.
