-1157441818 2014-06-10 There is no evidence of production equipment left behind and the land was properly restored. The location has met all 1509.072B restoration requirements.
1223436680 2013-07-15 Well was found idle at location. There is a tank with no dike uphill from the wellhead. The well is disconnected at the wellhead from the meter and separator which are found in same area as the well. There is no chart on the meter. Operator must construct dike around the tank in order to meet SPCC requirements, and operator must make well capable of production within 60 days; or plugging is required in 60 days. Dick Poling is sending in for a permit to plug this well in the next few days and plans on plugging the well sometime between now and fall.
1375815570 2013-08-06 I was on well to witness the bottom hole cement plug job by Wright Well Services. Formation arrived on location and set the plug from 3766 to 4166 with 45 sacks of Class A cement. They left location and Wright Services pulled the tubing. I returned when they had finished the last stand of tubing and they were rigging up to rip the pipe. Rip was unsuccessful and Dick was going to see if they had a cement bond log and possibly shoot the pipe.
1375886331 2013-08-07 Wright well services continued to try and rip pipe and had several blades break. Kenny then left to return the blades. He will call me when he returns to location. There was noone on location when I returned around 315pm. I called Dick and he said Kenny would have to be back today or tomorrow to rip the pipe.
1375987675 2013-08-08 Appalachian Wireline services came out to the location today and shot the 5.5 at 3172 however they could not get the casing out of the hole. Randy Wright spoke with Dan Goins who informed him that he could perf and squeeze the 200' lime plug, so they shot the casing for the plug at 2908-2912'. Randy is currently running tubing back into the hole.
1376062967 2013-08-09 I arrived on location for the Big Lime plug job. The bottom of the tubing was placed ~ 50' below the perf at 2957'. Formation Cement set a 200' plug from 2757-2957' using 58 sacks of Class A cement. Kenny was going to pull tubing out of hole and had Appalachian Well services line up for 1:00pm to shoot off the 5.5. The plan is to pull all of the 5.5 and run back in and spot the rest of the plugs on Monday.
1376316155 2013-08-12 Arrived on location to find Ken Wright setting up wellhead with hydraulic jacks. They shot the casing Friday and were unsuccessful getting the casing free from the hole. They then used two 1 Ton hydraulic jacks to pull the casing out and pressured up to maximum psi with no success. One of the valves on the jack broke so he left location to find a welder. He will call me when the resume work.
1376413722 2013-08-13 Ken unsuccessfully tried to pull pipe this morning and the cement bond log displayed a very tight hole most likely due to cave in from the Ohio shale. Ken was eventually able to get telegraph around 1650' and is now pulling the 5.5 casing from the hole. Formation cement is due on location tomorrow around 11:00am to finish plugging the rest of the well.
1376521472 2013-08-14 Today formation cement set a rip plug from 1434-1634 with 50 sacks of 2% Class A cement, a Berea plug from 1150-1365' with 58 sacks of 1% Class A cement, a surface casing plug from 628-828' with 51 sacks of 2% Class A cement, and a 200' surface plug with 102 sacks of 1% Class A cement. There was no circulation on the surface plug and the cement dropped down to around 350-450'. A brush and stone plug was set at around 204' and there was still no circulation. We will let the cement set over night and come back in the morning to tag the top in order to determine how much will be needed to plug the rest of the hole. 8/15/2013: Top of surface cement was tagged at 323'. The plan is to finish off this plug at the same time as the plug job for well 24657 at top of the hill.
1774749223 2014-06-23 Site meets 1509.072 restoration requirements.
1838163620 2011-06-30 Inspection this day found casing well in bottom, tank up at top of lease road, ID's say Poling, lacking most other information, no dike. Annual production shows the well is producing.
-2049126159 2013-08-22 I am on site today and witnessed that Wrights Well Service along with Formation Cementing Services set a 80 sack 330
528274534 2013-10-30 Site does not meet final restoration requirements. Operator has planted no seed in order to restore vegetative growth on the barren areas. There is still a 100bbl tank on location. I called Dick and left a message regarding the issues.
