The lease road is graveled and leads up to a fenced in pumpjack with house gas connection, proper ID signage and annular gauges. The tank battery has proper ID signage and is surrounded by a dike. The tank battery contains a 210bbl production tank, vertical mechanical separator, chart box and poly brine tank. There are no signs of leaks or mechanical issues for the tank battery or the wellhead. The tank battery, well and roadway conform for spacing requirements. Vegetation has been established. At this time preliminary restoration has been passed.
1420044590 2015-01-02 I was on location to check for preliminary restoration for this well. The lease road is graveled and leads up to a fenced in pumpjack with house gas connection, proper ID signage and annular gauges. The tank battery has proper ID signage and is surrounded by a dike. The tank battery contains a 210bbl production tank, vertical mechanical separator, chart box and poly brine tank. The annular gauge on the 11.75 and 4.5 diameter casing reads 165 psi of annular pressure. The 11.75 diameter surface casing was set at 461'. Assuming the formula of .303 x the depth of the surface casing the allowable pressure is 140 psi. Enervest has been instructed to bleed off the casing so that the pressure can be allowed to build back up and determine the static pressure of the casing. If pressure builds past the allowable pressure additional steps shall be taken. At this time preliminary restoration has not been passed.
1829045644 2015-03-17 I was on location to perform a status check of this production well. Inspection of the lease road's crossing of a stream found what appeared to be decaying crude oil on the stream banks. I contacted Doug Beighle of Enervest and requested staff to the location. The flowline of the well flows below the road and stream bed. The flowline was pressure tested at 30psi for 20 min and did not show any signs of leak. After investigation the substance was found to have orriginated from the drainage ditch lining the street (upstream from the stream and well). The substance is a biological decaying matter and not crude oil. I will follow up on the location tomorrow while the well is pumping to ensure no leaks are present.
2044430513 2014-06-18 I was on location to witness the cementing of the surface casing. A 14.75 diameter hole was drilled to 491' and 463
-243711417 2014-06-19 I was on location to check the location following heavy rain that moved through the area. The location is in good condition and the erosion controls are maintained. The Poulson rig is currently preparing to drill out the cement after waiting on cement for 8 hours. There are no violations at this time.
-34360230 2015-01-14 I was on location to check the well for preliminary restoration and the pressure of the annulus of this well. The pressure has been bleed off and is now 10psi, below the allowable limit of 140psi. Vegetation has not been fully established at this time and the a follow up inspection will be completed when the weather permits. There are no violations at this time.
-495688762 2014-06-24 I was on location to witness the cementing of the 10.5 lb/ft, 4.5 OD production casing. A 7.825 diameter hole was drilled to 5245' and new 4.5 OD casing was installed to 5195'. Centralizers were installed on casing joints 1, 3, 5, 7, 9 and 11. 150sks of cement (50/50 2% gel, 10% salt) were pumped, and displaced with 11 bbls of acid, and 70bbls of fresh water. The plug was landed, and the pressure was increased 500psi above final pump pressure and held for 5 min. The Poulson drilling rig will now be leaving Stark County.
530976030 2014-06-16 I was on location to check the status of site construction. Poulson drilling is on location but not fully rigged up. They are waiting on several loads for the rig before they can set in. Palmer Excavating was currently digging and preparing to line the pits. They expect to spud the well tonight.
