-1134976770 2015-01-31 While on location to witness the 9.625 casing cement job I was informed that 20 gallons of recycled water was spilled outside of the frac tank containment and 28 gallons were spilled insde the containment. The spill occurred while Stallion Services was loading drill water for disposal. A hose failure led to the release. The release consisted of fresh water used for drilling the intermediate section of the borehole. I prescribed that the water ( now frozen ) be mixed with the cedar dust and placed into the half round solids tank for disposal. Upon the completion of the cement job an examination of the area indicated that the previous spill had been remedied. No violation at this time.
1810202791 2015-02-02 I was on the Kiko # 5 - H to observe the status of the drilling progress. Sidewinder Drilling Rig # 63 was drilling a 8.75 @ 4083' KB. A walk around inspection revealed the previous ( 1/31/2015 ) release was cleaned up and no reoccurrences had taken place. A blind flange was missing from the SOBM tank # FT - 0029. I brought this matter to Aaron Wilhite ( R E Gas Development company man ) who had it replaced. No violations observed.
-1874675695 2015-01-31 I was on the Kiko # 5 - H well to witness the cementing of the 9.625 intermediate casing. Sidewinder Drilling Rig # 63 drilled a 12.25 borehole to 2,020' KB. KCS ran 52 joints of 9.625 casing to 2009'. Universal Well Services mixed and displaced a cement slurry comprised of Gas Block, 2% CaCl2, 0.15% CR - 3 through the interval 2009' to surface with 20 barrels of cement slurry returned back to surface. Mix water temperature was 54 F and pH was 7.
2091652321 2014-11-16 I was on the Kiko # 5 - H location to witness the placement and cementing of the 20 conductor casing. H A D Inc. drilled a 22.25 borehole to 110' GL with an Air Rotary drilling rig. H A D Inc. placed 108' ( 6 joints ) 20, 88 lbs. / foot, welded joint casing into the borehole. The conductor casing had circulation windows cut into the pipe up 5' from the bottom ( 103' ) for the placement of the cement slurry. Universal Well Services ( UWS ) connected to the casing and established circulation by pumping 10 barrels of fresh water ahead. UWS mixed and circulated a cement slurry back to surface. Mix water temperature was 39 degrees and ph was 7. The cement slurry consisted of 110 sacks of Type 1 with 3% CaCl2 ( blended ). I observed 5 barrels of cement slurry returned back to surface.
-600579127 2015-01-28 I was on the Kiko # 5 - H location to observe the cementing of the 13.375 surface casing on 1/28/2015. Sidewinder Drilling Rig # 63 drilled a 17.5 borehole from 120' to 1020'. The borehole made 1116 barrels of water from 763' to 1020' and was finished on fluid. The casing was placed in the borehole to 998'. Universal Well Services cemented the casing with 841 sacks of Class A blended with 3 % CaCl2, 0.25% lbs. / sack of celloflake. Displacement was achieved with 147 barrels of water and a top plug. Once the plug landed the pressure was increased to 1005 psi and held for 5 minutes without any loss. Ater bleeding the pressure off 1 barrel of fluid flowed back and stopped. All floats held. A walk around inspection revealed no violations.
