1097896989 2014-02-25 My inspection finds the well is equipped with 4.5 casing and 1.5 tubing. The tubing has a large hole in it at the top of the 4.5 by 1.5 wellhead. Gas can be heard leaking. The casing appears to have partially collapsed at the top of the 8 and is beginning to lean. A production unit is present off site. There is no tank for this well. The company reported gas production for the well in 2012, as well as 72 bbl of brine. Previous inspections have indicated the well is not capable of production. I called and left a message for John Snyder to call me to discuss this well at 12:06 pm.

1259784279 2009-10-21 Inspected site along with supervisor and determined that well is idle and incapable of producing in its present state of repair,it is equipped with 8' surface casing and 4.5' production casing and 1.5' tubing which has hole in it at tubing head,the last production reported was back in 1998 and I have had several conversations with company rep requesting that they plug or produce said well.
-1278567587 2014-08-04 My inspection finds the well is equipped with 4.5 casing and 1.5 tubing. The tubing has a large hole at the top of the 4.5 by 1.5 wellhead. The casing appears to have partially collapsed at the top of the surface casing head.
1415664647 2014-11-10 My inspection today finds Matheny Well Service is attempting to utilize a tubing spear to pull the 1.5 tubing out of the wellhead. The Stonebridge representative, John Schneider, stated that he believed the tubing has crimped a little at the wellhead slips and is preventing the tubing spear from entering the tubing enough to pull it. Another attempt will be made on Wednesday.
-1506484810 2014-09-22 My inspection today finds the status of the well has not changed. The well is still incapable of production. The 1.5 tubing has a large hole in it and the entire well is leaning. Gas is still escaping from the hole in the tubing.
1834179882 2013-07-11 I called and spoke to Mr. John Snyder. I told him that the plug back permit for the well would be expiring soon and that something needed to be done with the well. Mr. Snyder stated that he intended to begin work on the well in the next two months weather permitting.
-1877776063 2014-11-12 My inspection today finds the service rig crew has successfully used a spear to pull the tubing enough to strip the 4.5 by 1.5 tubing wellhead off. 2.875 tubing with a packer was placed over the 1.5 tubing. The packer was set and the tubing was then packed off in a wellhead on the 2.875 tubing string. The service rig was unable pull the tubing. John Schneider of Stonebridge Operating believes the tubing is stuck in the well at considerable depth.

2129607595 2014-02-28 I called Mr. John Snyder of Stonebridge Operating at 11:17 am. I discussed the outstanding violations against the well with Mr. Snyder. Mr. Snyder stated that he was planning to get a permit to plug the well. He also stated that the proper identification would be in place by 3/7/14.
-223774724 2013-11-06 My inspection today finds the well is equipped with 4.5 casing and 1.5 tubing. The tubing has a rotted off at the top of the well head. A significant amount of gas is leaking from the tubing. A production unit is present off site but is not hooked up. The well is not capable of production.
-2849950 2014-11-07 My inspection today finds the company has moved in a Matheny Well Service rig to attempt to repair the well. There are no personnel onsite. The tubing has broken completely off of the well and the wellhead has been partially partially disassembled. A valve on the 4.5 casing wellhead is open and a small amount of gas is flowing out of it.

939117533 2014-10-28 I received a telephone call from John Schneider, Production Supervisor for Stonebridge Operating, indicating that the company intended to make an attempt to repair the well in the next few days.
