1508220350 2013-03-12 I witnessed Nick's Well Plugging remove the pump jack and rods. They set the bottom plug through the 2.375 tubing from 4,572' to 4,872' using Class A cement. A total of 158 joints of tubing were removed. The 4.5 casing was ripped at 3,640' and 8 joints were removed. They broke circulation using fresh water for the #2 plug. It was set from 3,024' to 3,324'; placing it 50' above and 250' inside the Big Lime formation. They removed 63 joints of 4.5 casing. Circulation was broke using fresh water and plug #3 was set at 456' to 656'. They removed 11 joints and broke circulation. Plug #4 was set from 200' to surface. I witnessed a good show of cement returned to surface. A total of 88 joints of 4.5 casing was removed from the well.
1713821213 2006-11-21 no violations .
-1830062497 2013-02-19 My inspection found the well equipped with a pumping unit. There were small trees and weeds overgrown around the wellhead and unit. The tank battery contained one, 100 barrel production tank and one, buried, concrete brine tank. It was surrounded by a dike with overgrown weeds. The identification sign was slightly damaged. The separator contained a 7 day production chart. The lease road was in stable condition, but cover over with grass. The production records were up to date on our database. I called Gas Acquisition at 330-252-4560. I instructed them to clean up the small trees and weeds around the pumping unit. I will reinspect the site by 3/8/2013.
-2077991720 2013-10-09 My inspection found the all of the production equipment removed. The entire well site was graded and vegetation had been established. I witnessed Gas Acquisition power wash and crush the concrete brine tank on 10/1/2013. The pits were filled and all debris was removed. Waivers for the access road and flow line were obtained. Final Restoration passed.
2143156386 2013-10-01 I witnessed Gas Acquisition use Sutton Pump & Supply, Inc. to steam a concrete brine tank. The tank was thoroughly cleaned and all of the brine water was removed. The concrete brine tank was crushed and buried.
