2208531 2013-09-24 Violation(s) Noted On 9/24/13 at 11:05 AM SWN contacted the Department to report a release of approximately 105 gallons of ethylene glycol to the pad surface of the SWN RU 32 Folger Pad. Reported cause of release was a production equipment problem. Arrived on-site at approximately 12:30 PM. Mike Deming was on-site and explained that a well tender had identified the spill while responding to an automatic shut in of the Folger 2H (permit # 115-21140). The production unit which services the 2H had malfunctioned and overheated causing the fluid loss to the pad surface under the unit. The equipment failure caused the well and unit to automatically shut down. Equipment is being brought in to begin the cleanup of the spill. Explained to Mike that this case would probably be referred to the Departmentï¾’s EC&B Program. 679315 402CSL - Failure to adopt pollution prevention measures required or prescribed by DEP by handling materials that create a danger of pollution.
