1781736 2009-04-06 No Violations Noted Well site is equipped for production. Brine Tank located on-site is in sound condition. Following is a list of potential pollution concerns that were noted at the site:

1- There is a low-point on the access road that occurs directly above the culvert. Water will naturally pool in this area and could lead to erosion and sedimentation that flows directly into the stream. Additional BMP's should be implemented here. I would recommend installing rock at the low-point and adding additional silt fence around the pipe entrance and exit. 2- The temporary site identification tag has the wrong number on it.


1817414 2009-07-31 No Violations Noted According to records, well completed on 12/15/08 to a depth of 4,960 ft and WRAC submitted on 4/28/09. Active permit since 9/30/08. Well head completed with 7﾿ pipe reduced to 4﾿ and reduced to 2﾿ production line with 3 shut off valves. Production equipment consisted of heater, separator, data logger and 100 bbl tank with less than 20 bbl of brine. Line pressure on separator at 125 psi. No E&S issues and no IW issues during inspection surrounding the production well. The main access road to well will require additional E&S activity. Discussed topic with Penn View, Rick Blazek
1965275 2011-04-13 No Violations Noted At the time of inspection, well drilled and in production. Well completed on 12/15/08. Well tag present. Brine tank appears stable. Site has been restored with at least 70% uniform vegetative cover. Site does not exhibit accelerated ES issues nor IW issues, although detected a faint gas odor. Operator should continue to maintain shared access road accordingly, as new activity unrelated to this site is occurring at the time of this inspection.
2154437 2013-04-02 No Violations Noted At time of inspection, well in production. Well tag present. Brine tank structurally sound. Access road in good condition. Site stabilized with vegetation/rock. Ponding noted around production equipment. No accelerated E&S issues observed in relation to well site or access road.

Strong gas odor instantly noted with vapors visible over brine tank. Immediately referred to OGI Paul Wyrwas.
