2105000 2012-10-04 No Violations Noted The three wells at the site have been drilled. All three wells were labeled, but well 1H has the wrong permit number on its label. The 1H label should read 051-24490 and it reads 051-24465 now. This should be changed to reflect the correct well permit number. I understand the three wells are scheduled to be fracked late this fall.

Three pits were on the site and were all full of water. All three had the required over 2 foot of freeboard. There were three sed traps on the site. All traps looked to be functioning. All the E&S controls looked to be in place and were functioning. The stockpile on the site did not have any vegetative cover on it. The stockpile looked to be mostly comprised of rock/stone, which would make vegetative growth difficult. I did not see any major accelerated erosion off of the stockpile. I would suggest the operator keep a close eye on the stockpile to ensure no problems occur in the future with accelerated erosion. The only other equipment on the site was a dumpster for trash. The site was guarded. Site id sign in

2124994 2012-12-11 No Violations Noted ID sign up, wells labeled, E&S in place and functioning.
2184882 2013-07-18 No Violations Noted Site restoration extension requested by operator until 12/13/13. Site stable and 90% reclaimed at this time. Request approved.
2357539 2015-04-01 No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been restored as outlined in the PCSM plan. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
