1967796 2011-04-22 Violation(s) Noted Today Jon Scott and I inspected this gas well site in follow-up to Jon's 4/13/2011 inspection to determine if the operator had corrected the E&S concerns that Jon had told them about in a voice mail on 4/13/11. Upon inspection we found the operator had made some improvements, however the E&S controls remain in a state of disrepair. The slit fence is down in (2) locations on the north edge of the site and significant slit has left the slit traveling over 50 yards down slope. Also the slit fence is down on the east edge of the site with slit leaving the site as well. I called Jordan Florida, the local Rep for O&R Drilling and setup an on-site to review the problem for 4/25/2011. 609345 102.4 - Failure to minimize accelerated erosion, implement E&S plan, maintain E&S controls. Failure to stabilize site until total site restoration under OGA Sec 206(c)(d)
1967979 2011-04-25 Outstanding Violations - No Viols Req'd Today Scott Lux and I completed a follow-up inspection of this gas well site to determine if the operator repaired the E&S controls and slumping pit liner as required during our 4/23/2011 phone conversation, and to review what actions need to be taken to prevent further acclerated erosion. Upon inspection we found the operator had repaired the slit fence on the north edge of the site and also pulled up and secured the pit liner as required. However the slit fence remained down on the east edge of the site and the accerlated erosion is still occurring. After walking the site perimeter with the operator, the operator decided to eliminate vechile traffic on the well site access road and redirect the sheet flow from the fields on the west side of the site, to the access road. This work would be done after the operator installs (3) water bars to catch and hold the water on the road. Also the operaor will repair the down slit fence , and dredge/cleanout (3) sediment basins.
1972088 2011-05-09 No Violations Noted Today Scott Lux and I conducted a follow-up inspection of this site to determine if the violations identified on 4/22/11 have been corrected. Upon inspection we found the violations corrected.
1976681 2011-05-16 Violation(s) Noted Today's inspection was in follow-up to my 5/9/2011 inspection to determine if the E&S controls that have been installed have been maintained as required. Upon inspection I found the pits had been clsoed and the site remains unstable. The extensive nature of the earth moving for both the access road and the site , combine with the slope creates accerlated erosion that is being managed by water bars on the access road,sump pits on the site and slit fence. Upon inspedctiontoday i found the access road erosion being managed. However the site erosion was not. Sediment from the site has filed the sump and is overflowing with the water outflow, overburdening the slit fence. The slit fence is down once again on the north end of the ite and sediment has left the site. 1978012 2011-06-06 No Violations Noted Today's inspection was in follow-up to my 5/16 inspection and 6/1 phone conversation will company owner Ohannes Karaoghlanian to determine if the BMPS have been repaired as required. Upon inspeciton I found all slit fence reinstalled and all (4) sediment basins cleaned out. I also recieved the revised E&S plan as requested. This site needs reclaimed or steps taken to stabilze the soil must be taken; the pit area is not completely reclaimed.

Upon inspection I found the BMP are not being matined as required and the site reclaim work has not changed sincew my last inspection on June 6 2011. This even though i had cautioned the operator that he must keep the BMPs up and finish the reclaim to reduce potential problems. 619067 102.11 - Failure to design, implement or maintain BMPs to minimize the potential for accelerated erosion and sedimentation.

I met the dirt contractor on-site and inspected the old pit area and the new pit , before covering and closure. Upon inspection I found the work completed as required. The area still needs covered and stabilzed with seed and mulch.

2004619 2011-09-22 No Violations Noted Follow-up to determine if the access road and well pad have been reclaimed and stabilzed as required. Upon inspection I found the access road back to orginal grade with mulch and grass seed applied. The well pad site was also graded, mulched and seeded. However a top soil pile on the south edge of the site and (2) sumps on the north edge of the site remain on-site. I called the dirt contractor Jordan Flordia and he stated he left these in place intentionally because he expected the well gas pad site will be reworked when fracing for the utica.
