2073673 2012-06-05 No Violations Noted During this inspection, XTO rep Tom Baun and First Class Energy Rep( Construction Contractor) present. Site ID sign, ES plan, ESCGP-1 located onsite. Rig currently tore down, it is believed top hole drilled. Spud 5/22/12. minor changes have been noted on ES plan, the addition of a rock lined culvert along access road to well site and removal of culvert (per Penn Dot due to drainage on SR 410). Previously disturbed areas covered with matting and seeded/mulched with observed growth not quit 70% uniform coverage. Pad adequately rocked. No accelerated E&S issues on well pad. Temporary containment located onsite beneath former location of rig, appears sound. XTO reps and myself examined access road leading to a impoundment, not yet constructed. A culvert was installed and a sediment basin, it appears that due to grade and recent heavy rains the sediment basin could not handle flow; therefore, XTO would like to install a water bar leading to culvert located down gradient near well pad entrance. Dept ok'ed no increase in acerage or distrubed area-

During this inspection, Site ID sign, ES plan, ESCGP-1 located onsite. Rig onsite cementing and pressure testing. Previously approved minor changes noted on ES plan have been implemented, including rock lined water bar on impoundment roadway as well as rock lined ditches along access road entrance. Although it is recommended that operator maintain culvert outlets in order to prevent a possible pollution event.

Access road entrance has also been blacktopped since last inspection. Previously disturbed areas covered with matting and seeded/mulched with observed growth > 70% uniform coverage. Pad adequately rocked. No accelerated erosion or sedimentation issues noted on well pad. It appears sediment trap is properly installed and functioning as well as all temporary bmps'. Temporary containment located onsite beneath rig appears sound. No industrial waste concerns noted.

Bmp's installed and maintained, noaccelerated erosion or sedimentation concerns noted on access road or well site. Hence well site is located with HQ waters, department reiterated to operator that deflectors are not considered ABACT bmp's and in order to sustain sufficient sediment removal an additoinal ABACT bmp is reccomended, department referred operator to currentl ES manual for list of acceptable ABACT bmp's. Operator agreed to install water delfector in access road, as well as add additional compost sox in order to increase the effiencety of sediment removal. Based on this inspection, if operator installs/maintains additonal bmp's as outlined in ES manual and revised plans department shall ok minor changes as noted.

2128914 2013-01-08 No Violations Noted Access road entrance adequately rocked, although due to recent thaw and contiual traffic on/off site is reccomended that operator closely monitor and maintain access roadway as needed to ensure that mud does not exit the well site onto SR 210. Well drillled and equipped for prodcution. Well head surrounded by wire fence, recommend operator install a visible, permanent well tag on well head as per regulation. Prodcution equipment contained with in a metal containment unit. Brine tank appears sound. No evidence of Industrail waste concerns. Site snow covered; therefore, unable to adequately assess temporary stablization measures and temporary bmp functioning capacity. Although no accelerated erosion or sedimeantion issues observed. Department recommends that operator install a temp bmp beneath soil pile located on back side of well pad in order to prevent a potentail pollution event as a result of recent thaw, piles appears to be frozen. No violations noted at time of this inspection

At time of inspection, well equipped for production. Wellhead located in center of pad, surrounded by chain-link fence and cement barriers. Well tag present, legible. One on-site brine tank, appears sound. BT, production equipment located in lined secondary metal containment unit. Gravel, accumulated water within containment unit. Pad stabilized with rock. Downslope perimeters of pad vegetated. Area upslope of pad minimally vegetated, currently stable with aid of erosion blanket. Ruts observed around backside of containment unit. Department recommends Operator re-seed/mulch areas upslope of pad to ensure adequate vegetation capable of resisting accelerated E&S until final permanent restoration is achieved and also maintain areas of rutting to prevent any possible pollution events from occurring. Based on Operator's written request and this site inspection, an extension for site restoration will be granted until April 8, 2015.
