2013227 2011-10-21 No Violations Noted Routine inspection of site due to spud notification.

Pad was in final stages of construction. Four wells had been spudded. Drill pit was installed. Construction on impoundment was ongoing. Pad and access road were rocked and stable. BMPs in place. Toes of slope to be seeded asap. No violations noted.

2021169 2011-11-17 No Violations Noted SPUDDED, MOVING YOST RIG 6 ONTO PAD.
2026076 2011-12-12 No Violations Noted Site visit was part of routine inspection.

INspection was conducted with James Grayson (Safety Chevron). All equipment was on individual containment. Some areas where pipes laid on containmnet comprimised containment. No grease or oil stained the pad. Stands were being made to keep pipes from laying on walls of containment and causing them to collapse. Pit and impoundment were sound. No freeboard issues. Slopes of pad were not vegetated but had been hydroseeded. Other outlying areas of disturbed area had good vegetative cover. No violations noted at this time.

2026753 2011-11-29 No Violations Noted SPUDDED, MOVING YOST RIG 6 ONTO PAD.
