2037135 2012-01-24 No Violations Noted well site Id sign and revised ES plan present with emergency contact information on signage. ACRD stable. Multi well pad. Well drilled and equipped for production. Well spudded 4/07/10, drilling completed 8/25/10. Ten brine tanks located onsite which are surrounded by rock dike. Tanks appear structurally sound, although noted hose connected to main discharge line (manifold) of all tanks, it is recommended that operator remove hose when not actively emptying tanks in order to prevent a possible pollution event or even acts of vandalism. Bmp's installed according to plan and it appears that site has been stabilized with vegetative matting.no accelerated erosion or sedimentation issues noted on pad or access road, several sections of super/reinfoced silt fence that are structurally impaired-- therefore operaor should maintain. No violations noted at time of inspection.
2054688 2012-03-27 No Violations Noted Well sign(s) and tag(s) present. ES and ESCGP-1 onsite. Multi Well pad. Well(s) drilled and equipped for production. Drilling completed 3/15/11.

Since last inspection, residual waste roll off boxes have been removed. Well pad restored with approximately 70% uniform vegetative cover.

Bmp's installed and appear to maintained. Operator may remove temporary bmp's when "earth disturbance" is completed and vegetative growth is capable of resisting erosion. Wells actively being re-worked at time of this inspection. No accelerated erosion or sedimentation issues observed on site or access road. No IW issues noted.

Impoundment(s) actively being restored/backfilled not yet seeded or mulched. Reminder that operator shall restore grade and re-seed/mulch all previously disturbed areas including former location of soil piles. No accelerated ES concerns noted, bmp's are installed and being maintained.

2065692 2012-05-03 No Violations Noted Well sign(s) and tag(s) present. ES and ESCGP-1 onsite. Multi Well pad. Well(s) drilled and equipped for production. Drilling completed 3/15/11.
Well pad restored with approximately 70% uniform vegetative cover and rock.

Bmp's installed and appear to maintained. Operator may remove temporary bmp's when "earth disturbance" is completed and vegetative growth is capable of resisting erosion. No accelerated erosion or sedimentation issues observed on site or access road, although department reccomends additonal rock be added to well site entrance inorder to pervent a possible accelerated erosion and sedimenation concern, No IW issues noted.

Impoundment(s) restored/backfilled recently seeded and mulched with growth observed, although some areas may needed reseeded in order to maintain uniform 70% vegeatative cover. Temporary bmp's appear maintained.

No violations noted during this inspection.

2072689 2012-05-30 No Violations Noted
2081430 2012-07-06 No Violations Noted Well sign(s) and tag(s) present. ES and ESCGP-1 onsite. Multi Well pad. Well(s) drilled and equipped for production. Pad adequately rocked. Observed mini excavator onsite, not currently being utilized.

Drilling completed 3/15/11. Well pad restored with ~70% vegetative cover, although department is recommending operator reseed and mulch in the fall/spring in order to ensure that uniform vegetative cover is sustained. Temporary bmp's have been removed, department is also recommending that operator reseeded and mulch areas where bmp's were in place in order to prevent a possible pollution event. no accelerated erosion or sedimentation issues were noted on well site. Brine tanks were actively being pumped, no IW issues noted at time of inspection.

Impoundment has been removed and backfilled and properly seeded/mulched with observed growth. No accelerated erosion or sedimentation concerns noted surrounding impoundment, temporary bmp's have been removed.

At the time of this inspection no violations noted.
