2080828 2012-06-19 No Violations Noted On June 19, 2012, I stopped at the Chevronï¾’s Hambelton site for a routine inspection. The site has five stimulated wells but is not yet in production.

The site was in pretty good shape although I saw some erosion on the west slopes. The E&S plan was son-site.

2080861 2012-07-03 No Violations Noted viols from the 4/9/12 release (eFACTS #2065934) have been corrected.
2129957 2012-10-03 No Violations Noted I did a preliminary investigation and gathered background and historical information but was unable to sample because Mr. Hutchinson was not available to work the plumbing. I made preliminary plans to sample and left.
2130011 2012-12-06 No Violations Noted Scheduling conflicts made sampling difficult. On December 6, 2012 I again, stopped at the Hutchison residence and made arrangements to sample the water.
2130062 2012-12-12 No Violations Noted Scheduling conflicts made sampling difficult. On December 6, 2012 I again, stopped at the Hutchison residence and made arrangements to sample the water.
2130098 2013-01-14 No Violations Noted I received the various lab results and checked them against Moody's (Chevron's enviromental contractor) report. It does nto appear that the Hambleton gas wells impacted theHutchison's water supply. See attached report.
2219393 2013-10-24 No Violations Noted On October 24, 2013, I met with representatives from Moody and Associates to sample the Ed Nowak home water well and re-sample the Nowak industries/Black burn home water well (this well serves both the shop and the Blackburn home).

The Blackburnï¾’s have had issues with their water for quite some time but the Ed Nowak home well is a new complaint. We first sampled the Nowak home well. This well is buried in the front yard and is probably drilled, but the depth and the date drilled are unknown. Mr. Nowak treats the water with chlorine. Samples were taken before the pressure tank and are currently pending. We also took samples from the holding tank inlet just west of Nowak Industries (between the shop and the road). Moodyï¾’s techs found 19% LEL in the headspace above the well and 20.95 O2. These results are also pending. We attempted to take samples from the Blackburn residence but we could not get well water there because they are currently attached to a temporary water supply.
