1957110 2011-02-23 No Violations Noted 19:08 hrs ¿ received call from Jim Russell on site Operations Manager for Chesapeake that fire was in progress in the frac tank area and three workers were injured.

19:10 hrs ¿ received 2nd call from Mike Arch Oil and Gas Inspector supervisor of fire on a Chesapeake Well site.

19:55 hrs ¿ Oil and Gas Inspector Don Rugh arrived at entrance to well site. Access road is approximately ¾ mile long. Multiple fire companies were at site, perimeter control was in place and emergency equipment was being controlled from this point. At this time I started calls to Mike Arch with updates every 15 minutes. 20:15 hrs - Inspector Rugh was on well pad where undetermined number of frac tanks was on fire. Fire and emergency personnel were hosing down fire and adjoining tanks with foam and cooling sand and test separators with water.

All three wells on location 125-24033, 125-24034, & 125-24035 were shut in at the time of investigation/inspection. My purpose for being on location was to gather information from a technical perspective for the department's bureau of investigations. I observed no violations during the course of my investigation, and had the cooperation of everyone on location. My remarks on the following page detail my investigation. The following documents that I collected that day are in the possession of the bureau of investigations: Site roster at the time of the fire, tank strap information, and a timeline of events from spud through flowback up to the time of the fire.
