1374690222 2013-07-23 I witnessed Nick's Well Service plugged a well for Enervest Operating. Nick bullheaded the bottom plug with 30 sacks of Class A cement with 50 lbs. of Hulls. The operator displaced with 72 bbls of brine, then shut the well in at 2,000 psi for 1 hour. Plug #1 was set at 4,513' to 4,868'. The 4.5" casing was ripped at 3,459' and 10 joints were pulled. Nick's Well Plugging broke circulation with 20 bbls of fresh water and pumped 86 sacks of Class A cement. The operator displaced with 44 bbls of fresh water. Plug #2 was set at 2,776' to 3,076' to cover the Big Lime Formation. Next, Nick pulled 62 joints. The operator established circulation with 12 bbls of fresh water and pumped 60 sacks of Class A cement. The cement was displaced with 5 barrels of fresh water. Plug #3 was set at 320' to 520' to cover the bottom of the surface casing. Next, Nick pulled 7 joints. Circulation was established and 62 sacks of Class A cement was pumped. Plug #4 was set from 200' to surface. I witnessed a good show of cement returned to surface. A total of 84 joints of 4.5" casing was removed from the well.
1382618714 2013-10-24 My inspection found the well site graded and vegetation established. The pit was filled and debris removed. All of the production equipment was removed. I spoke to Enervest's Land Department in Hartville concerning the lease road; they had not received the waiver at this time. Inspection not passed.
