-1277215592 2010-06-18 An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. During the inspection, it was noted that the AD remains disconnected. Water drains from 100 bbl tank on hill (knockout) to tank battery located along Hysell Run Road. At the 100 bbl knockout tank location, a bare area extends approximately 50' x 10' below the tank to an earthen pit. A 10' x 10' area was also observed on the other side Approximately, ten dead trees consisting of young and older growth. A sample of the fluid was obtained to measure salt concentration with a High Range Quantab. The test was stopped at 7.8 units, which equals 3734 ppm (mg/L) Cl-. Notified pumper at location and left message requesting a return call. Mr. John Reese, Field Supervisor, would return call at 3:51 p.m. Mr. Reese stated that he was aware of pit and believed that it had been there for quite awhile. Further stating that a tank may have leaked at one time. Mr. Reese agreed to pump all fluid into tank, close pit and perform bioremediation. GPS reading obtained from this inspection, due to inaccurate well location in RBDMS. GPS reading also obtained for the tank battery. This case will be handed over to inspector, Jon Scott. 9-1-07
