174375808 2014-02-25 Inspect Well. Operator set up service rig on well to test the integrity of the 5.5 inch casing using the packer and tubing currently set in the well at various depts. Operator believed the perforations into the Utica Shale from 3788 feet to 3800 feet that were squeezed with cement prior to SWIW conversion were the cause of the pressure leak off on the packer and tubing. The packer was set and pressure tested at two depths above and below the perforations at 3762 feet and 3833 feet and loaded with water and pressured up to 1100 psi. There was no pressure loss at these depths. The Utica Shale Perforations are not causing the leak off. The operator set the packer and tested at two depth below a 5.5 inch cementing diversion (DV) tool set at 4100 feet. The depths tested were at 4143 feet and 4146 feet. The annulus was then pressured up to 1100 psi and shut in. The pressure rapidly leaked off. The determination was made that the DV tool has communication with the formation below 4100 feet and was not completely cemented off during installation and cementing of the 5.5 inch casing. A Cement bond log was performed July 11, 2013. the log indicates good cement above 4100 feet and no cement below this depth.

Top annulus with water and inhibitor. Pressure test well at 700 psi. Test Passed. Operator pumped 100 gallons of 15% hydrochloric acid into injection zone to clean up formation.

The packer was set and well pressure tested. The pressure test failed. The pressure leaked off due to foam and air bubbles. Testing rescheduled for next day.
