1349289882 2012-10-03 I inspected well and found a plunger lift system on the wellhead. I could not determine the tubing pressure or the casing pressure as there were no gauges installed on the wellhead. There was also a 50 bbl. steel oil storage tank installed next to the wellhead. I found no annular disposal piping on the casing and the 4.5 in. casing was shut in. The tank battery contained one 100 bbl. steel oil storage tank, one 50 bbl. steel brine tank and a chart recorder that had a chart that had not been changed since 02/01/11. The dike surrounding the tank was eroded and needs to have the height increased. There was an excessive amount of weeds within the tank battery and on the dike. The 100 bbl. oil storage tank was approximately 1/4 full of fluid. I discovered that the identification sign on the oil storage tank identified this well as the Smith R. #4 when it should be identified as the Giar-Neptune #4. I contacted the owner of S & S Energy Corp., Jason Scott, and advised him of the situation concerning the shallow dike and the improper lease name on the identification sign. He told me that he was not aware of the mislabeled identification sign, but would get out to the location and get it changed. He also advised me that he and his crew will be out in the area in mid October to do gas line tie in work and he would like fix the height of the dike at that time. I questioned him on the shut in and no production status of the well and inquired about possible plugging. He advised me that he wanted to start producing this well again by November once they tie in other nearby wells to Gatherco's lines.
1909754075 2014-08-04 Inspected production well on 08/04/2014. The tank battery consists of 1-50 bbl steel tank located within an earthen dike. The well is equipped with a wellhead with a swedge, gauge, and elbow. The well was shut-in at the swedge at the time of the inspection. No well identification is posted at either the tank battery or the wellhead as required. Research into the production history of the well showed that it has not produced since 2000, therefore the well must be plugged. 9-9-05(A)10
