12979460 2002-07-10 An unreported oil spill and an open unlined brine pit were discovered during a quarterly inspection of the injection operation. Inspectors Moravy and Worstall conducted the inspection. Also on site during the inspection was Jim Persinger (330-242-3539), King Oil pumper. The oil spill originated outside of the southeast corner of the tank battery dike. The vegetation kill zone affected an area approximately 100 feet long and 20 feet wide. At this point, the spill entered a small unnamed creek. King's response to the release was to lime and straw the vegetation and place sorbent pads in the drainage way. The pads were all oil saturated. King did not report the release to either OEPA or ODNR. The open pit was constructed for a pressure test conducted on January 25, 2002. After the testing, the pit was supposed to be pumped out and closed. Instead, the liner was removed and the pit was left open. Quantab analysis of the water in the pit indicated chloride concentrations in excess of 6,514 ppm. A new growth vegetation pattern indicates that the pit has overflowed and impacted downgradient vegetation. The facility was ordered closed and spill remediation was to begin immediately. Pete Mongue (330-242-3540) with King Oil was notified by phone.
1404320774 2014-07-01 One hour prior to inspection, the well tender pumped up the pressure on the annulus to two hundred pounds per square inch. An inspection found 75 psi. of pressure on the tubing and 197 psi. on the tubing/casing annulus. The pressure readings did not change, during a 20 minute period.
14379551 2002-07-19 Heartland Oil and Gas retained Petro Environmental to remove and dispose of the contaminated soil downgradient from the oil spill and the contaminated soil near the open pit. The pit has been pumped out and closed and the oil impacted soil has been removed. Both areas will be limed, seeded and mulched. The line leak was repaired and the loading pad vault has been pumped. The major issues have been addressed. The NOV will be satisfied after the vegetation has established itself. The receptor creek will need to be monitored for any sheen after the next several rainfalls. The stressed vegetation near the open pit will also need to be monitored. The on-site personnel included James Ray (480-816-6140), Pete Mongue (330-242-3540), and Jim Persinger (330-242-3539) with Heartland (aka Republic Oil) and Randy Jarvis (330-948-1494) with Petro.
