1356635031 2012-12-03 The Uniontown office was contacted by Dominion/EOG concerning a gas leak on this well. I arrived on location at 11:45am on 12/3/2012. Upon my inspection I found that a steel nipple ( 2" X 6" ) was corroded and leaking gas. Dennis Nething ( Pumper ) temporarily placed a pipe clamp on the affected nipple and stopped the gas leak. I was informed that Moore Well Service would be out to replace said nipple. The well was bypassed to the tank to relieve casing pressure. A crew from MWS was on location at 2:00 pm and effected the repair. The well was placed back into production. I inspected the location and found that the signage was out of date. I requested Nething to have the signs updated and remove the soil below the leaking nipple. 9-05E1
