1313193233 2012-04-17 Inspection of well found the following -8.625 surface casing -4.5 production casing -2.375 tubing -with valve on top in the closed position -production valves closed and lines un hooked , 210 barrel tank and separator present with no current chart or reported production since 1995,inspector will note well has lost radiation source in well bore -July 1984@6093' and plaque at well head , Inspector spoke with company man-Bill Arnold and he stated former producer placed packer on the 2.375 because of hole in casing and was unaware of source in hole and had plans to do up hole recompilation but will now further review their next action taken. Inspector asked co to properly identify well at the wellhead and tank battery and remount Radioactive plaque on a fixed assembly with their identification .
