Well operator has wrong PERMIT number on tank facility

1379516607 2013-09-18 The landowner, Dick Kessler, called to tell me there was oil in the dike due to a leaking tank. When I arrived, I found crude all around the front of a 50 bbl steel tank. The oil was leaking from holes in the side of the tank due to rust. I called Joe Altier and he said he would head over and clean up the mess. He will remove and replace the tank by next week. Joe called later in the day and stated that they had disconnected the tank and will begin cleanup. 9-1-07

1380219407 2013-09-26 Compliance Action Follow-up 1379516607:The bad tank has been disconnected and there is a new plastic tank off to the side of the access road. Most of the contamination has been cleaned up from dike. I will continue to monitor these issues until well is in compliance.
1942859601 2003-06-06 Inspection of the tank battery this date reveals there has been minimal effort taken to correct problems discussed with Mr. Altier on April 30, 2003. Oil is still present around the 2-100bbl tanks and also the area surrounding the seperator and meter. Mr. Altier was instructed to remove all oil contaminated material, removing the tanks to ensure all oil below them is removed; reconstruct a dike in compliance with SPCC regs. Mr. Altier advised the work would begin June 9, 2003 9-9-05(A)9
