-1037608483 2008-02-28 There is a small id on the control box. This is an Urban Permitted Site. This id is not in compliance with the permit conditions. I have called Rex Baker and he said that he is unaware of the urban id requirements. I informed him of the requirements of 1501:9-9-05(E)(1). There is a id on the tank that is for another well. Tank does not have a dike. I talked with Perry Brock (landowner) and he said that Bakerwell lost a bit and some rod in the hole recently while working on the hole. They lost it at 300' past the 90 degree turn. I have not been able to verify this information.
-1579330740 2007-04-05 Gas production line and equipment have been installed. Site restoration is not complete. Perry Brock the landowner is doing the work and he told me that the site would be used for a gravel stock pile so there was no need to seed the site. This has not happened yet. Gravel is only on a small portion of the disturbed land 75' by 100' the rest of the site is bare ground with trucks and trailers parked on it. I will follow up with Brock and Bakerwell. No erosion controls are in place at this time. The sediment fence was taken down when the pit was filled in.
-1815227307 2008-06-09 The first identification that is seen as you drive to the well site is an id on a 150 bbl tank that reads Johnson well #643. This tank is not even connected to the Brock well. Urban rules 1501:9-9-05(5)(b) covers the storage of unused equipment at a well site. This tank id will cause confusion during an emergency. The tank needs to be moved and an Urban id needs to be placed at the wellhead. No work has been done to comply with the previous NOV. I have called Rex Baker and left him a voice message that the id needs to be an urban id like the one that he used at the Fulks well. I also brought up the equipment storage violation.
-866579930 2009-12-17 Urban identification is located on a plastic 300 gallon tank. The steel tank is gone. Landowner has parked a dump truck 6 feet from the well. Along with other vehicles and debris. Took pictures.
