| 1307376191 | 2011-06-03 | The Uniontown office received a complaint in February 2011 from Jim Pastore regarding a drop in the water level in the lake on his property. Jim believes that the drop in the lake level is associated with the drilling and cementing operations conducted at the Schnase # 3 oil and gas well. The complaint was discussed on 2-16-2011 in the Uniontown office with Barry Lay (EnerVest), Rick Simmers (ODNR), Kyle Tharp (ODNR) and myself. Due to the flooding conditions at the wellhead, I was unable to gain access until 6-3-2011. I inspected the well constuction with Joe Bolinger (EnerVest) and Ron (Alborn Excavating). We excavated around the wellhead and checked the 4 dia shut off valves located on the annulus between the 16" dia casing and the 11-3/4 dia casing. These valves were used to cement off the artesian aquifer during drilling operations. The valves were found in the closed position. The west side valve could not be opened. The cementing nipple from the squeeze job to seal off the artesian flow was still screwed into the valve. The east side valve could be opened. There was no pressure on the valve and less than 1 quart of water trickled from the valve. The valve was left open and observed for 30 minutes. No pressure, no gurgling, and no flow was observed. All three of us agreed that there was no evidence of any aquifer flow observed. The excavated valves were placed in the closed position and buried. | ||||
| 1307471824 | 2011-06-03 | The drilling pits are closed and the well is in production. The location has been rough graded. Due to the flooding conditions on the site, the previously stacked sills and timbers are now scattered over the location. Remove all the timbers, sills, and debris from the location. Spread the stockpiled topsoil, grade, seed, and mulch the site. I did not pass the Preliminary Restoration at this time. | NONE5 | |||

| 1907102502 | 2012-01-06 | On 1/6/2912, I performed a status check on the above well site. The well is in production, the site is reclaimed and identification signs are posted. The Preliminary Inspection is PASSED. | ||||
| 1986462821 | 2011-07-18 | The Compliance Notice to restore the site is RESOLVED. |
