All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Well was equipped with a pumping unit and an electric motor. The lines running from the tubing and the production string were closed with ball valves. There was an additional 1" line running from the tubing directly into the 8" casing head. Both ends of that line had open valves. There has been no production recorded since 2008. Well shall be plugged or placed in temporary inactive status. The production line between this and the next well was severed and the end was in the creek. I saw no evidence of current contamination. Producer agreed to remove line from creek immediately. 9-9-05(A)10
