1487630059 | 2013-03-06 | Well is equipped for production as a gas well. Identification was observed at the well head laying on the ground. It is faded and needs re-written. There is a digital gas meter at the well head. According to landowner Mr. Gail Mayle, it was installed 2 years ago. It was not functional at the time of inspection. All valves were closed at the well head. There is no tank battery for this well. Production records showed 25 mcf of gas being sold in 2009, there is no record for 2010, and in 2011, 0 mcf of gas was produced. Well needs to be placed in production in 30 days or plugged in 90 days. | NONE

1765888752 | 2013-04-08 | When I arrived, there was a swab rig over the well and the crew was working on the well to get it back in production. Mr. Darren Ellis is the swab rig operator. I spoke with him regarding Broad Street's plans to put the well back in production.
323726317 | 2014-07-09 | Knox Energy pulled the 1.5" tubing and ensured it was clear and fit to set plugs through, then ran their jar down the 8.625" casing to tag bottom. They tagged at 1739', and the bottom was cement due to this well being plugged back in 1991. Cemenco was the plugging contractor on this job. Tubing was run back into casing to 1690' and 29 sks of 2% gel cement was pumped in to create a 100' bottom plug inside the 8.625" casing. Pulled 26 joints of tubing to 830', and set perforation plug from 830' to 630' using 61 sks of class A cement with 80 lbs of cottonseed hulls as LCM. Fluid level was gauged at approximately 400' after plug was set, and remained constant for 1 full hour. Circulation was not established on these plugs due to open perforations in the surface casing. Tubing was pulled from the casing and 6 bbl of fresh water was pumped into the casing to bring fluid level to 300'. A 300' plug was then set to surface using 58 sks of 2% gel cement. The cement dropped approximately 1' and stayed in place after 45 minutes of monitoring.
