2065730 2012-04-16 No Violations Noted On April 16 the Operator notified the Department at 6:30pm of a 25 barrel spill to the ground at the Stine 1 site in Loyalhanna Twp. Westmoreland County.
2065734 2012-04-17 No Violations Noted As a follow up to the spill reported by the Operator the Department was onsite 4/17/2012 to inspect the site. The Operator indicated at that time that a well tender reported an open valve on the brine tank associated with the well and immediately reported the spill to his supervisor who intern notified the Department. At that time of inspection the Operator started the cleanup process. The cleanup consist of excavating the contaminated soil directly adjacent to the contaminated are. Approximately 12 to 16 inches of soil lay on top of a red colored clay layer. Also excavated was a drainage ditch directly adjacent to the spill area. The Department observed 4 roll-off dumpsters were located on site, all appear be sealed and lined.
2065737 2012-04-20 No Violations Noted As a follow up to the spill reported on 4/16/2012 the Department observed that excavation that was done and the absorbent pads placed in the small drainage ditch and an underflow dam installed at the discharge point. The Department also suggest that the Operator install and interceptor ditch directly down slope from the spill area. The Operator indicated that water and soil samples were taken and the results will be forwarded to the Department.
2065739 2012-04-25 No Violations Noted As a follow-up to the spill incident of 4/16/2012 the Department met with the Operator to review the cleanup progress after the weekends precipitation event. The Department observed a very slight oil sheen on top of runoff water in the interceptor trench, the underflow damn and the drainage ditch. The Department suggest the Operator pump down the ditch and change all of the absorbent pads in the area. The Operator and Department will be continue to monitor the cleanup of this site.
2065749 2012-05-01 No Violations Noted As a follow-up to the spill event on 4/16/2012 the Operator informed the Department that after a precipitation event the following evening a sheen has been observed in the stream below the site. The booms and absorbent pad were already installed in the stream incase a release to the stream occurred. The Operator informed the Department that excavation will continue in the area directly adjacent to the stream below the site.
2065752 2012-05-07 No Violations Noted As a follow up to the spill incident on 4/16/2012 the Department met onsite with the Operator to discuss and review the excavation and clean up at the site. At the time of inspection a large ditch and underflow dam was installed below the site and approximately 5 feet from the stream. This area will continue to be monitored by the Department and Operator. At the time of inspecting all off the absorbent pads were recently replaced.
2103418 2012-09-27 No Violations Noted At the time of inspection the site had been regraded, seeded and mulched from earth disturbance measures put forth by the Operator to remove contaminated soil from the brine and oil release on April 16 2012. The Departments sample results the from 5/9/2012 were clean and at the time the Operator hired a private firm to assist in the remediation of the site. At this time the Department notes that the site has been regraded and seeded but does not meet the uniform vegetative coverage criteria. The Departments suggest the Operator continue to monitor the site weekly and after significant precipitation event to ensure the vegetative criteria is reached. The Department notes that Moody has supplied a report on the Operators behalf outlining all measures used to restore the site.
