The pit had approximately 3' of freeboard. At the time of inspection, pump trucks were on Site pumping out liquid from the pit for disposal.
During inspection, the drill crew was drilling 12-1/4" hole for 9-5/8" casing. TD for the 9-5/8" was scheduled for 2200'. The company man indicated that they had encountered a very small stream of water from approximately 50' and the hole continued to be wet from that point on with varying amounts of freshwater. Due to the excessive amount of freshwater encountered, the freeboard in the pit was being monitored, and pump trucks were being used as needed. |

During inspection, the drill crew was tripping drill pipe out of the hole. The company man indicated the reason was that the direction of the drilling was beginning to deviate too much, and a directional tool was on the way to the Site to straighten out the hole. The depth of drilling at the time of inspection was 4726'. Drillers log info: 1/2" stream at 50' and wet from there down, 2" stream at 420, 1" stream at 1680-1720 Casing: 13-3/8 set at 45', 9-5/8 set at 2200' with 1180 sks cement and 60 sks return.

| 1811405 | 2009-07-07 | No Violations Noted | On July 4, 2009, at approximately 9:30 a.m., I received a call from the Company Man at this location. He indicated that an incident had occurred at the site on the evening of July 3rd. According to the Company Man, the crew was in the process of plugging the hole. A plug was set from 6250 to 9000', and 2700 to 4700'. The crew had just set the second plug and reverse circulated the cement through the drill pipe. As the crew started to trip pipe out of the hole, the pipe-rams were mistakenly left shut, and the top of the derek was bent over as a result. One man who was on the derek at the time of the incident was injured with cuts to the face and broken facial bones. No there injuries were reported. A small amount of drill mud (< 1 bbl) was spilled on location. The Company Man indicated a small amount of hydraulic fluid was spilled onto the rig floor and was immediately cleaned up. No other spills were reported. | |
