A site ID sign and E&S plan were posted at the site. The access road was in good condition. All E&S controls were in place and constructed according to the plan at site. Pit was constructed. No spills, leaks, or accelerated erosion were observed at the time of inspection. The operator should continue to inspect the E&S controls until the site is restored and 70% vegetative cover is achieved.

Pit appeared to be in good condition and still contained fluids. No spills, leaks, or accelerated erosion were observed at the time of inspection. The pit liner appeared okay and the pit had more than 2 foot freeboard.

The access road was beginning to washout and the silt fence needed maintained. The access road must be stabilized and the sediment fence repaired or replaced.

2051715 2012-03-20 No Violations Noted On December 5, 2012, the Department received a request for a six-month extension for the reclamation of American Natural Resources’ (ANR) John Straka Unit #1. The well reached total depth on 4/1/11 and restoration should have been done by 1/1/12.

However, ANR was unable to complete the restoration on time because of the extremely wet summer and fall (they included documentation). I granted the extension and the new restoration date will be 7/1/12.

2085937 2012-07-24 Violation(s) Noted On March 15, 2012 the Department granted a six-month extension for restoration of American Natural Resources (ANR), John Straka Unit #1 site. The new restoration date was July 1, 2012.

On July 24, 2012, WQS John Ley and I stopped at the John Straka #1 site and found that (ANR) had not done the required restoration. Additionally, the cutting pit was is disrepair with the liner falling into the stored fluid.

This is a violation of Pennsylvania’s Rules and Regulations.

2100688 2012-09-06 Outstanding Violations - No Viols Req'd On July 24, 2012 the Department issued a NOV to American Natural Resources (ANR),for the John Straka Unit #1 site for missing the July 1, 2012 restoration deadline.

On September 6, 2012, WQS John Ley and I stopped at the John Straka #1 site and found the cuttings pit filled in, the site graded and re-seeded. However, we also found that grills and gullies were beginning to form. This is a violation of Pennsylvania’s Rules and Regulations.


Site restoration requires 70% perennial vegetation.
