| On site a produced fluids tank ruptured due to the plumbing of  the relief valve into an  unvented  tank.  When the relief valve relieved presure to the tank the presure caused the tank to swell then rupture.  Rupturing of the  produced fluids tank caused a release of multiple barrels of  crude oil and brine water mixed to disperse over the site and down over an embankment into the waters of the Commonwealth.  WQS S. Steingrabe is onsite gathering water samples.

| The drill pit that had been encapsulated during well development  had been located and and shown to the small excavator operator for avoidance.  Excavation was in process to remove and delineate the affected area in the immediate vicinity of the well site.  Subsuequently excavation will be undertaken along the pathway that brine/oil traveled to the tributary to Sewickley Creek to remove the affected media.  According to Mr. Elkin, Moody & Associates personnel will be onsite later today, 07/20/15, to begin background and confirmation sampling.  WQS Higgins suggested that Atlas Resources consider utilizing a vac truck to remove contaminated fluids from the tributary to Sewickley Creek, especially if significant precipitation is expected, both before and after the rain event.  There was a a light sheen observed in tributary both before and after the underflow dams.  This is likely attributed to oil and brine that had been deposited on the vegetation and streambanks during the recent high water period.

| Site inspection performed as part of the follow-up to the crude oil/brine release.  Site excavation. confirmation sampling and background sampling had been completed on 07/20/15.  The excavated area immediately around the well-head and tank left onsite was left open to allow for additional excavation should sample results indicate more excavation is necessary.  Temporary erosion controls had been implemented in this area.  The area affected by the breakout flow of the brine/crude and precipitation had been excavated, confirmation and background samples taken, regraded and temporarily stabilzed with erosion control blankets.  The underflow dams implemented in the UNT were functioning properly.  There was little to no sheen observed in the UNT.  The small wisps of sheen observed were likely attributable to product that had been absorbed by the vegetation along the stream banks.

| Site inspection found that the site was in much the same condition as was observed in the 07/22/15 inspection.

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1736764 Primary Facility 09/12/2008 Drilling/Alteration No Violations Noted Well spudded. Location/pit/freeboard/E&S controls Ok.
2389917 Primary Facility 07/15/2015 Routine/Complete Inspection Repairs or Upgrade Required On site a produced fluids tank ruptured due to the plumbing of the relief valve into an unvented tank.

2390827 Primary Facility 07/20/2015 Follow-up Inspection Outstanding Violations - No Viols Req'd The drill pit that had been encapsulated during well development had been located and and shown to the small excavator operator for avoidance.
2393192 Primary Facility 07/27/2015 Follow-up Inspection No Violations Noted see hard copy for remarks
