| On December 20, 2011, an estimated 320 gallons of hydraulic oil was released to the soil by a malfunctioning line on a gas drilling rig.  Chief provided a remediation report to DEP on March 9, 2012.  On April 11, 2012, DEP received additional information from Chief’s consultant.  The case was closed on April 13, 2012.  The drilling company did not seek an Act 2 release from liability.

| I arrived onsite at 1445 to perform a site restoration inspection in response to an NOT submittal received by the Operator.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, several sections of infiltration berm 1 have not attained adequate vegetation and contain exposed erodible material. The rest of the site appears to have attained adequate vegetation.

The Department requests that the Operator inspect infiltration berm 1 for any exposed erodible material and deploy stabilization measures in accordance with your site plan.

2035400 Primary Facility 01/11/2012 Incident- Response to Accident or Event Violation(s) Noted On December 20, 2011, an estimated 320 gallons of hydraulic oil was released to the soil by a malfunctioning line on a gas drilling rig. Chief provided a remediation report to DEP on March 9, 2012. On April 11, 2012, DEP received additional information from Chief’s consultant. The case was closed on April 13, 2012. The drilling company did not seek an Act 2 release from liability.
2316790 Primary Facility 10/16/2014 Complaint Inspection Pending The Department (Andrea Mullen, Eric Rooney, Jon Sassi, and I) met with Jon Hafer (Chief Oil & Gas) on site at the Squier Unit Well Site to collect soil samples in response to a nearby water quality complaint. The Department collected the below referenced samples to be analyzed with the SAC 961 with the addition of sulfates, lead, calcium, bromide dry weight, and lithium :

2851123 Primary Facility 2019-03-01 Site Restoration No Violations Noted I arrived onsite at 1445 to perform a site restoration inspection in response to an NOT submittal received by the Operator.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, several sections of infiltration berm 1 have not attained adequate vegetation and contain exposed erodible material. The rest of the site appears to have attained adequate vegetation.

The Department requests that the Operator inspect infiltration berm 1 for any exposed erodible material and deploy stabilization measures in accordance with your site plan.
