I receiving a phone call from Paul Shaw, Dorfman's Production Supervisor, that ARG had ran their tank truck over while loading oil from their production tanks and that 5 +/- barrels was on the ground. Clean up has been completed all contaminated stone and soil has been removed and disposed of. The tank battery has two 100 barrel tanks and one horizontal separator. The well has a pumping unit with an electric motor. All ID signs are posted. No violation noted at this time.

| I conducted a status check on this site and found that the well is equipped with a pumping unit and connects to a tank battery that consists of two 100 barrel stock tanks and a separator.  There was no brine storage tank. The tank battery is shared with permits 0246 and 1800. The thief hatches were closed and there were bull plugs installed in the draw valves. The dike was free of vegetation and debris. Proper identification was posted. I found no leaks or spills at the well head or the tank battery. I took new GPS coordinates on the well head and tank battery. No violations at this time.

| On 8/13/2013, I was in route to the New Philadelphia Field Office, when I received a phone call from Paul Shaw Production Manager for Dorfman Production stated that ARG Oil Hauler was connected to the values loading oil from their 2- 100 barrels stock tanks on to their truck when the driver noticed that his truck tank was running over. Paul estimated about 5 barrels +/- was on the ground. Dorfmans water truck was there vacuuming up the oil. He said that Sweet Meadows was on location to dig up and remove all contaminated soil and properly disposal of. ARG personal were also responding to the area to assisted in the cleanup. I told Paul that I would be on location 9/14/2013 to inspect the location. This tank battery serves (3) wells (1)- 3409920240 - (2) 3409920246 -(4) 3409921800
