A panel of the sound wall has been removed to tie-in the gathering pipeline spur and will be replaced within the next few days once line is complete. Fraccing is estimated to take place around mid-March. The site is currently snow covered; however it appears that all E&S controls remain in place and are being maintained. The site appears to be stable and no violations were noted during this inspection.

• Temporary stabilization was found in place on all disturbed areas on site. • PCSM control implementation was completed at the time of inspection. • Disturbed areas along the adjacent pipeline were not stabilized at the time of inspection. No violation.

2837937 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted On 2/5/19 a complaint inspection related to the Mad Dog 2020 Well Site was conducted in response to a reported water supply impact. The following was observed:

>>The complainant reported recent third party testing indicated multiple elevated parameters. Sample 3401-080 was collected from an outdoor tap per standard procedure.
