On 04/14/16 at approximately 0830 hours I conducted an incident response inspection in response to a spill that occurred on 04/13/16. The Act 9 sign was in place and all required documents were present at the site.

On 04/13/16 a spill of 2/3 freshwater and 1/3 reuse water occurred. Approximately 55 barrels were recovered from containment. Chevron discovered that fluid leaked to the ground surface through a bad seam approximately 8” up the containment berm. After the fluid was recovered from the pad surface the faulty seam was repaired. The spill area off containment was excavated and the material placed on a liner and covered to prevent rain infiltration. Confirmatory samples were taken after excavation occurred. The spill volume to the ground was calculated at 30 gallons.

2463022 Primary Facility 2016-03-08 Routine/Complete Inspection No Violations Noted On 03/08/16 at approximately 0830 I conducted an incident response inspection of the PBG Smith Unit. On 03/07/16 a spill of approximately 120 barrels of frack fluid occurred. The spill was reported as contained on secondary containment.

The Act 9 sign was in place and all required documents were present in the mailbox. The wells on site are currently being fracked. The containment appeared to be sound and appeared capable of containing the volume of fluid reported as released, due to a high berm on the edge of the containment. There were no signs that any fluid left containment. It is recommended that the area be inspected after containment is removed to verify that no fluid was outside of containment.

2467432 Primary Facility 2016-03-24 Administrative/File Review No Violations Noted

On 03/24/16 at approximately 1050 hours I conducted an administrative inspection in response to a spill at the Chevron PBG Smith well site.

On 01/18/16 at approximately 0930 hours friction reducer was being circulated and a hose became disconnected allowing approximately 130 gallons of friction reducer to be spilled to containment. Operations were immediately shut down to prevent the additional release of fluid. The area was cleaned up in compliance with DEP spill policy.

2479455 Primary Facility 2016-04-14 Incident- Response to Accident or Event No Violations Noted

On 04/14/16 at approximately 0830 hours I conducted an incident response inspection in response to a spill that occurred on 04/13/16. The Act 9 sign was in place and all required documents were present at the site.

On 04/13/16 a spill of 2/3 freshwater and 1/3 reuse water occurred. Approximately 55 barrels were recovered from containment. Chevron discovered that fluid leaked to the ground surface through a bad seam approximately 8” up the containment berm. After the fluid was recovered from the pad surface the faulty seam was repaired. The spill area off containment was excavated and the material placed on a liner and covered to prevent rain infiltration. Confirmatory samples were taken after excavation occurred. The spill volume to the ground was calculated at 30 gallons.

On 06/06/16 at approximately 0900 hours I conducted a CEI in response to an NOV issued to Chevron Appalachia LLC for a discharge of approximately 30 gallons of brine fluid that passed through a bad seam in the containment and contacted the ground.

The Act 9 sign was in place and all required documents were present in the mail box. The location of the spill had been excavated, filled, seeded, and mulched. Vegetation had begun to grow in the disturbed area. There were no signs of additional spills or discharges at the time of inspection. There were no violations observed.

The Act 9 sign was in place and all required documents were present at the site. The site was well stabilized and there were no signs of erosion observed at the time of inspection. A small area off the southwest edge of the pad exhibited thin vegetation. It is recommended the area be seeded and mulched. A minor erosion rill was observed off the southwest edge of the pad in the direction of the basin. It is recommended the area be addressed, seeded, and mulched.
