| I witnessed the cementing of the conductor casing.  Express Energy Services drilled a 26" diameter hole to 120'.  Express installed 120' of 20" OD casing.  During drilling, the borehole got damp at an approximate depth of 80', but did not produce much water.  Allied Oil & Gas Services was the cementing contractor.  The cement slurry was mudscaled at 15.6 pounds per gallon (ppg).  Allied pumped 52.4 bbls of cement slurry at 15.6 ppg.  The cement was displaced with 39 bbls of water.  An estimated 15 bbls of cement slurry returned to surface.  Displacement was completed at 1:28pm.

| I stopped by the site to check on the status of well completion and restoration.  The well has been hydraulically fractured. At this time a work over rig is installing 2-7/8 tubing.  Production equipment is on site but not hooked up.  I didnot walk the entire site due to the active. All erosion and sedimentation controls, and drainage controls appear to be functioning.  Ground cover is well established along the access road and around the drilling pad.  No areas of erosion are present.  Preliminary restoration approval will not be finalized pending installation of production equipment.

| I was on site to assess the completion of preliminary restoration.  The pad is shared between #21805 (Judy Brown #3H) and #21823 (Judy Brown #6H).There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. Identification is also posted at the wellhead and tank battery. The access road has a padlocked gate near Haessly Road.  The access road is in good repair, and drainage and erosion controls are in place.  Ground cover is well established along the access road.  I found the production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate.  The fence is in good repair.  The pad has a crushed limestone surface, and is in good repair.  The well is constructed with a
