Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1767160 2009-01-27 No Violations Noted Well staked, site not constructed at the time of the inspection. After phone conversation and e-mail correspondence with the operator representatives, the E&S plan was revised to address the following deficienies: 1) Depth to seasonal groundwater table makes the use of land-farming, encaspulation, and below-grade pits not possible. 2) The culverts located on the access road for existing stream flows require BMP's so as to isolate them from the disturbed acreage. 3) The entire access road and site should be rocked so as to minimize the amount of sediment generated during well development.
1808525 2009-06-19 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Well was not equipped for production. Below grade pit located onsite and had more than the two feet of required freeboard. E&S plan implemented as outlined in the approved E&S plan. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. The operator should continue to monitor the site for accelerated erosion & sedimentation until well development and final restoration activities are completed.
1886550 2009-11-02 No Violations Noted Drilling completed. Site not restored yet. Tag located. No off-site erosion & sedimentation. Pit is still on-site and is in sound condition.
1886561 2010-04-07 No Violations Noted Drilling completed. Site not fully restored yet. Tag located. No off-site erosion & sedimentation. Pit is still on-site and is in sound condition. Roll-off box on-site.

Contacted operator representative Doug Miller to ask when site would be restored. - Because the Drill cuttings are being hauled to a land-fill they are waiting for final approval from waste management before they can finish restoring the site and remove the roll-off box.

1927311 2010-08-24 No Violations Noted Drilling completed. Well is equipped for production. Tag located. Brine tank appears to be in sound condition. Vegetation is well established. No off-site transportation of erosion & sedimentation. At this time, the site appears to be restored.
