| File review reveals Chesapeake has submitted the waste manifests as had been requested by the Department for the 9/29/11 ethylene glycol spill which occurred at the Welles 1 3H Pad.  As was identified in the Departments 4/16/12 inspection, Chesapeake had met the residential cleanup standards for soil, thus Chesapeakes response to this this is deemed complete.

| Arrived onsite for a routine inspection at 09:38; the weather was ~35°F and sunny. The gate was open when I arrived.

There were 2 properly tagged Marcellus wells staged for production. A small amount of water was in the well cellars; no bubbling was observed. There were also 2 GPUs, 2 sand separators, and 1 double-walled produced water tank.

Areas around the pad were greater than 70% vegetative cover and no E&S issues were noted. An infiltration area was constructed as depicted on PCSM plans and appeared to be functioning as intended.

There were 2 properly tagged Marcellus wells staged for production. A small amount of water was in the well cellars; no bubbling was observed. There were also 2 GPUs, 2 sand separators, and 1 double-walled produced water tank.

Areas around the pad were greater than 70% vegetative cover and no E&S issues were noted. An infiltration area was constructed as depicted on PCSM plans and appeared to be functioning as intended.

The old steel produced water tanks and secondary containment were removed, however, the fencing for the tanks remained in place.
