| On June 2nd, 2011 I conducted a routine inspection of the above site. Drilling and frac were completed and the well was now in production. A well tag was present. A site ID sign and E&S plan were posted at the site. The access road was in good condition. All E&S controls were in place and constructed according to the plan at site. The pits were closed and the site was rough graded. No spills, leaks, or accelerated erosion were observed at the time of inspection.
