2776118 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted Weather was 72 degrees f. and raining at time of inspection. Met with Alta representative Ian Andrews on location. Well was a spud conductor. There was a steel plate covering the casing. The mouse hole was also spud and covered with a steel plate at time of inspection. No violations were noted at time of inspection.
2778839 Primary Facility 2018-09-24 Routine/Complete Inspection No Violations Noted Weather was 60° and cloudy at time of inspection. Met with Alta representative Bill Mitchell on location. Well was closed in with a night cap at time of inspection. The Surface and Intermediate casing had been drilled and cemented prior to this inspection. Reviewed the cement job logs from the surface and intermediate casing and no discrepancies were noted at time of inspection. Rig will move back over this well to complete the production section after the other wells on location are finishing being top holed. Conducted a walk around inspection of location. No violations were noted at time of inspection.

Rig was in the process of drilling the 8 3/4" hole for the production casing at time of inspection. They were expecting to reach their kickoff point tomorrow (11/8/18) sometime. No issues were discussed with drilling at that time. No violations were noted at time of inspection.
2804551 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted Weather was 36° and cloudy at time of inspection. Met with Alta representative Bill Mitchell on location. Rig was in the process of pulling drill pipe. Drilling to TD was completed at 12am today. They intend to pull drill pipe and circulate today and run casing at approximately 7am 11/20/18. No issues were discussed with drilling at that time. No violations were noted at time of inspection.
2807326 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted Weather was 34° and cloudy at time of inspection. Met with Alta representative R.D. Mann on location. At time of inspection rig had completed the production casing cement job. Reviewed the job log from the cement job and no discrepancies were noted at time of inspection. No violations were noted at time of inspection.
2807802 Primary Facility 2018-11-28 Drilling/Alteration No Violations Noted The site currently has the FlexRig 225 on location. The rig recently finished drilling the 081-21744 well and will be moving off location soon. The rig manager was informed of a small hydraulic oil spill off containment. The Department recommended the site be cleaned up asap. The operator/rig manager cleaned up the hydraulic spill within the next few hours. The spill is believed to be less than one gallon because the contaminated soil and rock was equal to 1 barrel.

Violations noted but immediately corrected.

2817401 Primary Facility 2018-12-19 Routine/Complete Inspection No Violations Noted Weather was 26° and sunny at time of inspection. Met with ALTA representative Bill Mitchell on location. Well was drilled and cemented to TD and closed in with a night cap. Well was covered with rig matting at the draw-works at time of inspection. Will follow-up with a visual inspection when rig moves off location. Rig was in the process of running production casing to TD on the Alden Evans 4H. No violations were noted at time of inspection.
2826070 Primary Facility 2019-01-07 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. There were no Alta representatives on location at time of inspection. Well recently had 3 strings of casing drilled and cemented to TD. Rig had moved off location to the Mac Pad. Well was shut in with a night cap to wait for stimulation. Well was covered with a steel grate at time of inspection. Well cellar was clean and 0% combustible gas was detected at time of inspection. The mouse hole was open and not yet filled with gravel. No violations were noted at time of inspection.
