The perimeter controls were in place and the site did not meet the restoration criteria of 70% uniform perennial vegetative converge. The site appeared to regraded, however the Operator has failed to restore the site within 30 days of the well permit expiration.
2415088 Primary Facility 2015-10-13 Follow-up Inspection Outstanding Violations - No Viols Req'd Follow-up inspection to the violation issued on 7/9/2015, see inspection # 2386865. In the violation response letter received by the Department on 7/23/2015, the Operator indicated that "WPX has removed all of the silt soc". This is contrary to the observation made during this inspection. The well site is not restored. The site does not meet the 70% uniform perennial vegetative restoration criteria and the perimeter BMPs remain in place. The Department suggests the Operator reseed and much the well pad area. Once 70% uniform vegetation is established, remove the silt soc. The Department also suggests the Operator continue to monitor the site weekly and after significant precipitation events
2434269 Primary Facility 2015-12-18 Follow-up Inspection Violation(s) & Outstanding Violations Follow up inspection to violation issued on 7/09/2015. See inspections #2386865 and #2415088. At the time of inspection the well was not drilled. The permit expired on 5/29/2015. The perimeter controls were removed; however large areas of the lacked 70% uniform vegetative coverage. The well site ID sign was removed and the Erosion and Sedimentation plan could not be located. The Department suggests the Operator replace the ID sign and place a copy of the E&S plan on site upon receipt of this inspection.
2490995 Primary Facility 2016-06-03 Compliance Evaluation No Violations Noted Compliance evaluation inspection in response to violations issued on 7/09/2015, see inspections #2386865 and #2415088. At the time of inspection the well was not drilled. The site has been restored. A notice of termination was submitted on 5/12/2016 and approved on 6/3/2016. The well site ID sign was replaced and the E&S plan was located on site. The violations have been corrected.
