| I arrived @ 09:00 hrs.  SWN 44 rig on location.  I spoke with company man Rick Sizemore; he informed me that they had lost the curve and lateral on the 4H well, caused by faulty casing.  During cementing, it became apparent that there was a hole in the casing.  They had set a balance plug and were skidding the rig, but plan to come back to the 4H afterwards.  Containments were in good shape, chemical storage area and rig backyard were well maintained.  I departed @10:00 hrs.

Currently, two work-over rigs are on site and the majority of the pad surface is covered by containment. The electrical conductivity of puddles outside of the containment was screened and ranged from 275 to 495 uS/cm. No sheens or evidence of spills was observed outside of containment.
 
 
 
Numerous puddles with petroleum sheen were observed within the containment area. A vacuum truck is on site working to remove the puddles, both within and outside of the containment.

| The operator submitted a well site restoration report which prompted this inspection. Four out of four permitted wells have been drilled and stimulated. These wells were producing at the time of the inspection.
 
 
 
The site is stable with greater than 70 percent vegetative cover. A bioretention berm and rock-lined channel provide PCSM. These appear to be functioning as designed. The site has been permitted to remain at full pad size.
 
 
 
Some materials associated with adjacent pipeline construction are being stored by another contractor on the pad.
 
 
 
This site meets the post-drilling requirements under 78a.65.

| At 1240 on 10/8/18 Mike Deming of SWN reported a spill of approximately 10 to 15 gallons of produced water to the surface of the Cupper well pad to the 24-hour emergency line.
 
 
 
I arrived on the Cupper pad at 1229 on 10/9/18. I met there with Mike Deming and two other SWN employees. He showed me the location of the spill which was next to the containment for the produced water tanks. He informed me that contaminated soil had already been removed from the surface at the site of the spill and placed in a roll-off tank that was still located on site at the time of my inspection. Mr. Deming told me that before removing the soil, they were getting conductivities of 1.5mS/cm to 2mS/cm where the spill occurred. The affected area of the pad was covered with a tarp. The pad surface under the tarp appeared slightly darker in color than the surrounding pad surface but appeared to be almost completely dry. Mr. Deming said that they believe that the spill likely occurred at some point when the produced water tanks were be

| At 1240 on 10/8/18 Mike Deming of SWN reported a spill of approximately 10 to 15 gallons of produced water to the surface of the Cupper well pad to the 24-hour emergency line.

I arrived on the Cupper pad at 1229 on 10/9/18. I met there with Mike Deming and two other SWN employees. He showed me the location of the spill which was next to the containment for the produced water tanks. He informed me that contaminated soil had already been removed from the surface at the site of the spill and placed in a roll-off tank that was still located on site at the time of my inspection. Mr. Deming told me that before removing the soil, they were getting conductivities of 1.5mS/cm to 2mS/cm where the spill occurred. The affected area of the pad was covered with a tarp. The pad surface under the tarp appeared slightly darker in color than the surrounding pad surface but appeared to be almost completely dry. Mr. Deming said that they believe that the spill likely occurred at some point when the produced water tanks were be

Grading of the site is currently underway. Perimeter erosion and sediment control BMPs are in place and in good repair; no signs of accelerated erosion or sedimentation were observed. Temporary stabilization was in place on disturbed ares that are not actively being worked.

No violations were cited during this inspection.

2592156 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted I arrived @ 09:00 hrs. SWN 44 rig on location. I spoke with company man Rick Sizemore; he informed me that they had lost the curve and lateral on the 4H well, caused by faulty casing. During cementing, it became apparent that there was a hole in the casing. They had set a balance plug and were skidding the rig, but plan to come back to the 4H afterwards. Containments were in good shape, chemical storage area and rig backyard were well maintained. I departed @10:00 hrs.
2599551 Primary Facility 2017-06-01 Routine/Complete Inspection No Violations Noted Inspection to assess site conditions during drilling operations. I arrived on site at 08:30 hrs. Southwestern representative Joseph Pendergraft was present during the inspection. No violations cited.
2632677 Primary Facility 2017-09-06 Routine/Partial Inspection No Violations Noted Inspection to assess site conditions after well stimulation. I was accompanied by Ed Blystone of Southwestern.

Currently, two work-over rigs are on site and the majority of the pad surface is covered by containment. The electrical conductivity of puddles outside of the containment was screened and ranged from 275 to 495 uS/cm. No sheens or evidence of spills was observed outside of containment.

Numerous puddles with petroleum sheen were observed within the containment area. A vacuum truck is on site working to remove the puddles, both within and outside of the containment.

No violations were cited during this inspection.

2647001 Primary Facility 2017-10-13 Routine/Partial Inspection No Violations Noted Inspection to assess site conditions during flowback operations. Much of the containment used during drilling and completion activities has been removed. Containment is in place around the well heads, brine tanks, and other equipment being used for flowback operations. Puddles on the well pad surface were screened for electrical conductance and ranged from 225 to 450 uS/cm. No evidence of spills or releases off of containment was observed. No violations cited.
2662541 Primary Facility 2017-11-20 Routine/Complete Inspection No Violations Noted Weather 36 Deg. F, cloudy and windy. Well has a digital gauge with 598 PSI production pressure. Unable to conduct visual cellar condition. Able to test for gas in cellar, 0% combustible gas detected at the time of inspection. No annulus vents observed during inspection.
2694261 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted Weather was 33° and sunny at time of inspection. Well head is covered with a large orange cage. Water tanks are placed on location to supply water to the Brown pad for fracturing procedures. There was one analog pressure gauge attached to the wellhead above the swab valve. It read approximately 400 psi at time of inspection. No violations noted at time of inspection.
2775237 Primary Facility 2018-09-11 Site Restoration No Violations Noted The operator submitted a well site restoration report which prompted this inspection. Four out of four permitted wells have been drilled and stimulated. These wells were producing at the time of the inspection.

The site is stable with greater than 70 percent vegetative cover. A bioretention berm and rock-lined channel provide PCSM. These appear to be functioning as designed. The site has been permitted to remain at full pad size.

Some materials associated with adjacent pipeline construction are being stored by another contractor on the pad.

This site meets the post-drilling requirements under 78a.65.

2787743 Primary Facility 2018-10-09 Incident- Response to Accident or Event No Violations Noted At 1240 on 10/8/18 Mike Deming of SWN reported a spill of approximately 10 to 15 gallons of produced water to the surface of the Cupper well pad to the 24-hour emergency line.

I arrived on the Cupper pad at 1229 on 10/9/18. I met there with Mike Deming and two other SWN employees. He showed me the location of the spill which was next to the containment for the produced water tanks. He informed me that contaminated soil had already been removed from the surface at the site of the spill and placed in a roll-off tank that was still located on site at the time of my inspection. Mr. Deming told me that before removing the soil, they were getting conductivities of 1.5mS/cm to 2mS/cm where the spill occurred. The affected area of the pad was covered with a tarp. The pad surface under the tarp appeared slightly darker in color than the surrounding pad surface but appeared to be almost completely dry. Mr. Deming said that they believe that the spill likely occurred at some point when the produced water tanks were be

2806671 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted Weather was 36° and snowing at time of inspection. Met with Southwestern representative Jonathan Woodburn one location. Well was shut in due to drilling of the 5H and 10H on location. Visually inspected wellhead, cellar and surface discharge equipment. Well had digital pressure monitoring equipment attached to the production tubing. Tubing pressure was 336 psi at time of inspection. Well was covered with a large orange steel frame during drilling operations. No violations were noted at time of inspection.
2816046 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted Weather was 37° and cloudy at time of inspection. There were no Southwestern representatives on location at time of inspection. Containment crews were on location removing rig matting. Well pad recently had 2 wells drilled and cemented to TD. Conditions of this existing well appear to be unchanged or unaffected by drilling at time of inspection. Well had a digital pressure gauge attached to the pay line at time of inspection. No violations were noted at time of inspection.
