2206457 Primary Facility 2013-09-19 Routine/Partial Inspection No Violations Noted An inspection was conducted at the Hurler Pad in Harrison Township, Potter County on September 16, 2013. The site is in the process of being restored; the pad and impoundment have been restored to original contours. The site is well vegetated, but was still <70%. There was some erosion channels forming, but no sediment had yet left the site. A representative of Penn Virginia was contacted and a plan was initiated to address the issues. A follow-up site visit was conducted on September 19, 2013 and a subcontract was on-site stabilizing the site. No new violations were cited during this inspection. There are six drill permits associated with this site: 105-21758 - 1H (Proposed But Never Materialized), 105-21783 - 2H (Operator Reported Not Drilled (ORND)), 105-21787 - 3H (ORND), 105-21785 - 4H (ORND), 105-21808 - 1H (ORND) and 105-21827 - 1H (Active).
