| Landowners were worried eabout landslides from drilling/blasting.  Well is 0.35 miles horizontally and the leg is one mile beneath them.  I found no problems.

| On March 12, 2012, WQS John Ley and I stopped at CNX’s NV-32 pad to check the E&S controls.

The first thing we found was an oil sheen on the access road at the pad’s entrance; probably from a backhoe at the entrance.  Although a contractor and a security guard were on-site, no effort was being made to clean up the spill.

We could not find a copy of the Erosion and Sedimentation plan.

There were some E&S controls (silt sox) on-site but they did not always seem to be placed where they could do the most good. The finished slopes were not stabilized.

We did find where sediment had left the pad.

These are all violations.

| On August 28, 2012, I stopped at CNX’s NV-42 site for a Compliance Evaluation Inspection; I met with Spenser Johnson.

| style="text-align: right;" | 0.00727

+
| style="text-align: right;" | 3661.01
CNX has drilled all the wells on NV-42 vertically and is waiting horizontal drilling.

The slopes and sides are vegetated.  Although some areas are a bit spares, I consider the slopes stable.

The E&S plans are on-site and it looked like they installed them according to those plans.  CNX placed added a rock pad to the access road’s bank and we added that to the plans.  Some of the storm water ditches were showing signs of erosion and I suggested adding some extra rock to them.

I found no problems during this inspection.

| On May 16, 2018, the Department performed an inspection of the HG Energy, NV42HS well site. Present at time of inspection was Jared Stemple and Jay Kendra (HG).  The site is located in South Franklin Twp., Washington Co.  The inspection was related to a request by the operator to meet on site to discuss a slid which has developed on the site along the access road and start of construction of the PCSM structures.
 +

 +
The operator plans to re stabilize the fill slope along the access road which recently failed.  After the slide repair the operator plans to begin installation of the PCSM structures on the site and progress termination of the ESCGP2 permit.
 +

 +
5 wells have been drilled to completion on the site and are equipped for production.

| On 11/8/2018 at approximately 1200, I conducted a Compliance Evaluation Inspection of the NV42HS well site located in South Franklin Township, Washington County. A file review was conducted by John Carson and David McDermott (DEP) on 11/8/2018 regarding a spill of drilling based mud to the graveled well pad. It was determined during the review that a Compliance Evaluation Inspection was required in order to close out the incident. The following was observed during the 11/8/2018 inspection:
 +

 +
* Act 9 sign located at the site entrance was in good condition.
 +

 +
* All GPUs on site were not within secondary containment but appeared to be in good condition. Wells on site were put into production prior to October 2016.
 +

 +
* All wells on site were marked and appeared to be in good condition.
 +

 +
* Temporary stabilization measures were utilized surrounding site in relation to a major modification of the ESCGP. The modified area appeared to be stable and no sediment appeared to leave the site.

* Indications of slumping at th

2738318 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted On May 16, 2018, the Department performed an inspection of the HG Energy, NV42HS well site. Present at time of inspection was Jared Stemple and Jay Kendra (HG). The site is located in South Franklin Twp., Washington Co. The inspection was related to a request by the operator to meet on site to discuss a slid which has developed on the site along the access road and start of construction of the PCSM structures.

The operator plans to re stabilize the fill slope along the access road which recently failed. After the slide repair the operator plans to begin installation of the PCSM structures on the site and progress termination of the ESCGP2 permit.

5 wells have been drilled to completion on the site and are equipped for production.

2801410 Primary Facility 2018-11-08 Routine/Complete Inspection No Violations Noted On 11/8/2018 at approximately 1200, I conducted a Compliance Evaluation Inspection of the NV42HS well site located in South Franklin Township, Washington County.
  • Act 9 sign located at the site entrance was in good condition.
  • All GPUs on site were not within secondary containment but appeared to be in good condition. Wells on site were put into production prior to October 2016.
  • All wells on site were marked and appeared to be in good condition.
  • Temporary stabilization measures were utilized surrounding site in relation to a major modification of the ESCGP. The modified area appeared to be stable and no sediment appeared to leave the site.
  • Indications of slumping at th
